Social Services
Primary indicators for Social Service Intervention would include:
- Patients with suspected safety and/or welfare concerns
- Patients and/or caregivers voicing Stressors / concerns / complaints / anxiety
- Patients and/or caregivers demonstrating cognitive or mood changes
- Patients and/or caregivers requesting diagnosis information/education
- Patients and/or caregivers requesting/demonstrating need of alternate placement arrangement (i.e.: assisted living or long term care facilities)
- Patients and/or caregivers requesting/demonstrating need for in-home services (i.e.: home health, Meals on Wheels, homemaker services)
Patients and/or caregivers demonstrating difficulties with compliance, decision making, long range planning and setting of realistic goals or expectations
