Social workers in outpatient settings deliver services that assist patients & providers. The key functions and tasks performed by the social workers include:

  • Assistance with care coordination involving home health & other referrals
  • Assisting patients & families with locating financial and/or medication assistance
  • Assistance with referrals to specialized community services such as medical transportation, senior support services, food assistance, housing issues, etc.
  • Educating patients on how to apply for federal, state & local assistance programs
  • Educating & assisting with Advance Directives & Living Will forms
  • Assessment & intervention in suspected abuse or neglect situations
  • Assessment for need of emergency mental health interventions
